Do you accept insurance?
Please know we are considered an out-of-network provider and still work with your insurance. By scaling back on accepted insurances, we are able to invest in top of the line equipment and products, thereby providing you with the highest quality and standard of care. As a courtesy, our staff will complete and process all insurance claims and submit them to your insurance company, along with all necessary treatment information.

What's your cancellation policy?
Please provide 48 hours notice if needing to cancel or reschedule an appointment by calling our office. Multiple missed appointments may lead to dismissal from the practice.
